Thank you for choosing PCP For Life as your primary care physician. Our offices are open 7 days a week with extended hours to take care of your healthcare needs. With our practice, you have the choice of being served at any of our eight locations, with any of our 20+ providers for your health care. We look forward to see you!

How to Get Started?

If you are unable to complete the paperwork for any reason, be sure to arrive at least 15 minutes before your appointment to complete your needed paperwork.

New Patient Packet

Download our new patient forms, available below for you, to fill out and bring to your appointment. We also ask existing patients to fill these out each year to keep demographic information updated.

What's in New Patient Packet?

  • Form 1: Demographic Information, Medical Release and Insurance Information
  • Form 2: Basic Health Information – Family History, Concerns, Habits, Medications and previous care
  • Form 3: HIPAA Notice and Privacy Practices

What All Should You Bring?

For your visit, we recommend that you bring:

  • Valid photo ID (Driving license, Passport, etc)
  • Insurance Card or Proof of Insurance
  • Filled patient packet (for new patients or first visit in a new year)
  • Details of Prescription medications – Name, Dosage, etc.
  • Any other test results or medical documentation that you feel would help us take care of your healthcare needs
    (eg: ultrasound results, x-ray results, previous medical records, details from other specialist etc.)

What to Expect?

  • During your initial appointment, our front staff will verify your insurance to make sure all information is correctly entered within the system and check you in.
  • Our medical staff personnel will call you within 15 minutes to cover any allergies you may have, gather the reason for the visit, and prep you up for the visit that includes taking vitals, weight, any questionnaire or exams that may be needed for the visit, and have you seated in a room.
  • The provider will visit you and follow the required procedures or take needed exams based on your medical history, current concerns or care requirement
  • The medical staff will cover any additional documentation, care, injections, refill, etc. as approved by the provider.
  • The front staff will finalize the visit and reschedule you for your next visit based on provider’s request

What Should be Yearly Appointment Schedule?

As your primary care physician, our providers prefer to see each patient at least once every 3 months or more often based on the healthcare needs of our patient. We plan and set below appointment schedules for all patients:

  • One Annual Physical visit per year
  • One Well Women Exam visit per year (for female patients)
  • Four quarterly visits to cover regular health checks
  • Specialized visits for Medicare patients in regards to:
    • Diabetic Eye Exam
    • Mammogram Visits (Female only)

You can reschedule or cancel any of these visits, however, as your partner in health, we strongly recommend to follow above schedule each year at a minimum.

How we handle Cancellations, Rescheduling and Arriving Late?

We understand there are times when you must miss an appointment due to unexpected circumstances. However, when you don’t call to cancel an appointment, you may be preventing another patient from getting much-needed treatment.

There will be no charge for first three missed appointments. A $25 fee may be charged for any no-show appointments after that. This will not be covered by your insurance company and must be paid prior to rescheduling.

If you are delayed and arrive more than 15 minutes past your scheduled appointment time, we may have to reschedule your visit for the next available time to ensure the safety and prompt care of every patient.
